Every child entering a New Jersey school is required to have a physical examination by a doctor. The state also requires proof from your doctor that the child has received certain immunizations before entering school. These requirements are 4 DPT, 4 Polio, and 2 MMR (Measles, Mumps, Rubella). The last doses of DPT and Polio must have been given after the child's fourth birthday. In addition a varicella vaccine is required or documentation of having had the chicken pox and the hepatitis series must be completed. The full date - day, month, and year - is required for each immunization. Children transferring in from out of state or out of the country may need a Mantoux test as well.
