The Health Office works hand in hand with the family, to support the child and student in school. Please contact me with any changes and updates in your child's health. Telephone or in person conferences are encouraged at anytime during the year.
In order to support safety and the state requirements for attendance, if your child will be absent from school, you MUST leave a message on my voicemail 201-848-5982 or send me an email at [email protected]. Please leave a detailed message every day your child is absent, with the reason your child will not attend school, by 9:15am.
Health Forms:
Forms are available below to print and bring to your physician/health professional for completion. Please complete the parent portion, filling in your child's name, your contact information and signature (at the bottom) before submitting the forms to the physician.
PLEASE NOTE
~Both Parental/Guardian and Physiciansignatures are mandatory, and must be present before medication is administered at school. This is the law.
~Medication must be supplied from home, in the original container, in an unexpired state.
